- Palliative Education
- Financial and Payer education
- Hospice Benefit Education
- Hospice Del Sol services education
- Counseling around issues of death, dying, and grief
- Community Resources Education
- Advanced Directives Education
- Social Services can also assess the patient and family unit for emotional factors relating to the terminal illness. They will also assess for the need of environmental resources, communication barriers, positive support systems, and caregiver need for respite. This assessment will contribute to the comprehensive assessment that allows of the optimal plan of care to be developed.
- Social services can also provide assistance with funeral planning through support and guidance.
- Patient and family questions and concerns can be promptly addressed through a social worker. And the appropriate support will be provided.
