Palliative Education Program

Studies prove that having access to the right information and resources helps reduce hospital readmission and lengths of stay. More importantly, it adds to the quality of life and may even extend it.

What is Palliative Care?

Palliative care is a comfort care model that helps relieves stress caused by serious illness, improving the quality of life for the patient and his family. It is available for people of any age and at any stage of illness, whether that illness is curable, chronic or life limiting.

Many private insurance companies including HMO’s offer palliative care benefits. Medicare part B also offers some benefits and Medicaid benefits vary by state.

The goal of palliative care is to make the patients comfortable. For longer-term palliative care outside of the hospital setting there is Hospice care. Hospice care becomes the most appropriate choice when other treatments and procedures no longer add to the quality of a person’s life.

Path Program Includes

  • Quality of life must consider the physical, material, social, emotional, and spiritual well-being, as well as, the dignity and autonomy of each patient.
  • Assistance in understanding the disease process.
  • Addressing the social, spiritual, emotional concerns of the patients, their families, and primary caregivers.

Path Benefits Includes

  • Better coordination and continuity of care
  • Greater understanding of patient’s needs and resources available outside of the facility
  • Overall improvement in the patient’s quality of life, greater peace of mind
